We invite you to initiate contact with us by completing our online form if you or a family member want to become a new client AND:
willing to receive email/text exchanges from our practice to initiate services; you may opt out later
able to receive and complete intake paperwork within 72 hours after initating consultation with our practice
have availability during the following times to schedule the first and/or ongoing appointments
Monday to Thursday 9am-12pm
Alternating Friday & Saturday 9am-12pm
We have limited availability and are only processing potential new clients who availability aligns with available appointment times.
Upon receiving your form submission, our practice will initiate a secure chat consultation to discuss you or your family member's needs to determine if our services are a good fit! You will receive an email to initiate a secure chat with our practice!
If it is deemed we are unable to meet you or your family's needs, the secure chat account will be made inactive, and you may receive a referral to another practice/provider based on the information you provided.
We value your privacy and confidentiality throughout this process and will handle your information securely and professionally.
